Horizons Mental Wellness LLC provides secure telehealth psychiatric care for adults and teens across Louisiana. This online scheduling option is designed for patients who are ready to complete intake paperwork, submit required documents, and request an appointment through our streamlined onboarding process.

Please be prepared to upload the following during the online intake and scheduling process:

• Front and back of your primary insurance card
• Front and back of any secondary insurance card(s), if applicable
• A valid government-issued photo ID

Completion of these items is required for appointment review and insurance verification.

Please note Horizons Mental Wellness LLC is not enrolled as a Medicaid provider. If Medicaid is your primary or secondary insurance, you may be referred to a participating provider.
