Cristina Evans is a board-certified Psychiatric Mental Health Nurse Practitioner providing psychiatric evaluations, medication management, and treatment for conditions commonly affecting Veterans including PTSD, anxiety, depression, and ADHD. Telehealth appointments allow Veterans throughout Louisiana to receive professional mental health support without needing to travel long distances.

VA Community Care Network (VACCN) Mental Health Referrals

Cristina Evans, PMHNP-BC  participates as an in-network provider with the Veterans Affairs Community Care Network (VACCN) Region 3 and provides telehealth psychiatric care for Veterans across Louisiana. Veterans who receive care through the VA may be eligible for community care mental health services when approved by their VA care team.

Community care services require authorization from the Department of Veterans Affairs before appointments can be scheduled. Veterans typically obtain this authorization through their local VA Community Care coordinator or referring VA provider.

Veterans who wish to explore community care mental health options may discuss available providers with their VA care coordinator or referring VA provider.

Appointments are provided through Horizons Mental Wellness via telehealth when authorized through the VA Community Care Network.
