Cancellation & No Show Policy
This policy outlines appointment cancellation, rescheduling, late arrival, no-show, and intake paperwork expectations.
Late, Cancellations, and No-Show Policy
A 15-minute grace period is alloted for late arrivals. If a patient has not joined the session within 15 minutes of the scheduled start time, the appointment will be canceled and considered a no-show.
If you need to cancel or reschedule, please provide at least 48 hours’ notice. Cancellations or no-shows within 24 hours will be charged 100% of the service fee regardless of insurance status. This fee is not billable to insurance, including Medicare, and is the patient’s direct responsibility.
You will be notified of the fee, and if not paid within 5 business days, the card on file will be charged automatically, in accordance with our financial policy.
Repeated missed appointments interfere with continuity of care. Patients who accumulate three (3) missed appointments/no-shows or late cancellations may be subject to discharge from the practice.
In the event of discharge, appropriate notice and resources will be provided to support continuity of care, as required.
Paperwork Submission Policy
All required Intake Forms/Consents must be completed at least 24 hours prior to your appointment. Failure to complete required paperwork may result in cancellation or rescheduling of your appointment.
